Overview
The Injury Fund is completely funded by registration fees paid by all playing members, as well as some non-playing members and therefore it is essential that all monies to be reimbursed be prior notified to the Fund and approved, to ensure that sufficient funds are in place
As per Rule 82 , Prior Approval is required for all treatments outside of:
o 90% of the first 6 physiotherapy sessions after injury
o GP Visits
o A&E visits (including emergency attendance to VHI Swiftcare, Laya Health & Wellbeing Clinics, Mater Rapid Injury Clinic etc) up to a maximum €100
Procedure
For all private appointments or treatments not listed above (including scans, consultations, surgery, additional treatment), I wish to clarify the process as follows:
- Preliminary Claim form to be submitted to within 8 weeks of injury date in hardcopy or by email by the County Secretary to injuryfund@lgfa.ie
- Claim will be opened under the Injury Fund and a claim number issued via email to the player.
- Referral for private treatment to be sought by player by way of a signed/stamped letter on business headed paper from the attending medical Practitioner (medical doctor or physiotherapist).
- Referral letter to be submitted in hardcopy or by email by the player to injuryfund@lgfa.ie quoting claim number, player name and a request for prior approval.
- Prior Approval will be issued via email to the player indicating same has been granted.
- Steps 3 – 5 to be repeated for all further private treatments.
- Once the Full Injury Fund Claim Form and relevant documents are received at the end of treatment, all Prior Approval requests are checked against any receipts received for private treatment.
- Only those receipts which have a corresponding prior approval on file can be reimbursed.
non-compliance will result in non-payment of costs to players.
